Elder Care Support Program

The Elder Care Support program supports Elders and older Aboriginal and Torres Strait Islander people to understand the aged care system, including identifying the level of care and services required.

Our Community can receive greater local support, linking services with local knowledge, to access the right aged care services to meet their needs.

How our free Elder Care Support service works for you:

The Elder Care Support service helps connect Elders and people over 50 in our Community with the care and services they need.  

A number of Aboriginal community controlled organisations now have a specially trained Elder Care Support Connector who knows and understands our people in Community.

  1. The Elder Care Support Connector can come and meet you at your home or wherever you feel safe and comfortable - with your family or carer if you prefer. There’s no cost to you or your family. We can even come and pick you up for our yarns.
  2. When you meet the Elder Care Support Connector, you can yarn about things that would make you feel more supported in your home. Maybe some help with your cleaning, shopping or cooking or help with more personal care? It’s up to you!
  3. Our Elder Care Support Connector then does all the work to make sure you are connected to the aged care services and support that you need. If things change for you and you find you need some extra or different help, they will arrange that for you too. Or if you’re not happy with the providers of the service, the Elder Care Support Connector will help get things back on track.

Our Elder Care Support service now makes it easier to receive aged care support and care you need.
